Anaphylactic shock — drug of choice; alpha-1 action reverses hypotension, beta-2 action relieves bronchospasm, and beta-1 action supports cardiac output, simultaneously reversing every major life-threatening component of the reaction.
Cardiac arrest — beta-1-mediated increase in heart rate/contractility and alpha-1-mediated vasoconstriction (raising coronary/cerebral perfusion pressure during CPR) help restore spontaneous circulation.
Added to local anesthetics — alpha-1-mediated vasoconstriction at the injection site reduces systemic absorption, prolonging local anesthetic action and reducing bleeding.
Acute severe asthma (refractory to inhaled bronchodilators) — beta-2-mediated bronchodilation, given when inhaled therapy is inadequate or inaccessible.
