Chosen drug: Lidocaine
Justification: Lidocaine is a Class IB antiarrhythmic that selectively suppresses abnormal ventricular automaticity (ischaemic/depolarized tissue) with minimal effect on normal conduction, making it the drug of choice for acute ventricular arrhythmias following myocardial infarction. It has a rapid onset (IV) and short duration of action, allowing easy titration in the acute setting. Propranolol, while useful in post-MI care for its beta-blocking and antiarrhythmic properties, is not the first choice for treating an established acute ventricular arrhythmia immediately post-MI, and its negative inotropic/chronotropic effects can be hazardous in the acute peri-infarct period with haemodynamic instability.
