Adrenaline is the drug of choice in anaphylactic shock because its combined alpha-1 and beta receptor actions directly reverse every life-threatening component of the reaction: alpha-1-mediated vasoconstriction reverses the profound hypotension from vasodilation and capillary leak; beta-2-mediated bronchodilation relieves bronchospasm and laryngeal edema; beta-1-mediated positive inotropic/chronotropic action supports cardiac output; and it stabilizes mast cells, reducing further histamine release. Given intramuscularly (anterolateral thigh) for its rapid, reliable onset.
