Answer

Finasteride inhibits 5-alpha reductase, the enzyme that converts testosterone to dihydrotestosterone (DHT), the androgen primarily responsible for prostatic growth. By lowering intraprostatic DHT levels, finasteride causes gradual shrinkage of the hyperplastic prostate gland over several months, relieving the static (anatomical) component of bladder outlet obstruction in BPH — complementary to alpha-blockers (e.g. prazosin, tamsulosin), which relieve only the dynamic (smooth muscle tone) component without reducing gland size.
