Answer

In thyrotoxicosis, excess thyroid hormone increases tissue sensitivity to catecholamines, producing marked sympathetic overactivity (tachycardia, tremor, anxiety, sweating, heat intolerance). Propranolol, a non-selective beta blocker, rapidly controls these symptoms by blocking β1-mediated cardiac effects, providing prompt symptomatic relief while definitive antithyroid therapy takes effect. It also inhibits peripheral conversion of T4 to the more active T3, providing some direct antithyroid benefit, making it particularly useful in thyroid storm.
