Liposomal Amphotericin B is the preferred choice — the lipid formulation substantially reduces the off-target renal tubular binding responsible for conventional amphotericin B’s dose-limiting nephrotoxicity, while preserving (and in visceral leishmaniasis, actually improving, via preferential uptake by the reticuloendothelial system where the Leishmania parasite resides) antileishmanial efficacy. This favorable safety profile allows shorter treatment courses and has made it the WHO-recommended first-line agent for visceral leishmaniasis, over the more nephrotoxic conventional formulation.
