Oxytocin is the appropriate first-line choice — it produces controlled, effective uterine contraction to control postpartum hemorrhage with a favorable safety profile, and is not contraindicated in hypertension. Ergometrine causes sustained vasoconstriction and is contraindicated in hypertensive/pre-eclamptic patients, given the risk of a dangerous further rise in blood pressure; it is reserved as a second-line/adjunct agent for atonic PPH refractory to oxytocin, in normotensive patients.
