Question

Answer

Chosen drug: Oxytocin

Justification: Oxytocin is the first-line drug for both prevention and treatment of postpartum haemorrhage due to uterine atony, given its rapid onset, short duration of action allowing easy titration, and favourable safety profile (minimal cardiovascular side effects at standard doses). Ergometrine, while also effective for uterine atony, causes sustained tonic contraction and carries a higher risk of hypertension and vasospasm, making it contraindicated in patients with hypertension/pre-eclampsia and generally reserved as a second-line agent when oxytocin alone is insufficient.
