Explain the rationale for the use of oximes in organophosphorus poisoning.

Answer

Oximes (e.g., Pralidoxime) act as cholinesterase reactivators. Organophosphates phosphorylate and inactivate acetylcholinesterase; oximes have a strong affinity for the phosphorus atom of the OP-enzyme complex and remove it, regenerating active acetylcholinesterase. This reverses both the muscarinic and, importantly, the nicotinic (neuromuscular) effects of OP poisoning — which atropine alone cannot address. Oximes are most effective if given early, before “ageing” (a further chemical change that makes the enzyme-inhibitor bond permanently resistant to reactivation) occurs.
