Answer
Ondansetron is a 5-HT3 receptor antagonist.
MOA: ondansetron → blocks serotonin (5-HT3) receptors both peripherally (vagal afferents, relevant to enterochromaffin-cell serotonin release from chemotherapy/radiation mucosal damage) and centrally at the chemoreceptor trigger zone.
Uses: chemotherapy-induced nausea and vomiting (first-line, combining peripheral and central blockade), postoperative nausea/vomiting, radiotherapy-induced emesis.
Adverse effects: headache and constipation (5-HT3 receptors also mediate normal GI motility), QT prolongation (a class-wide caution, requiring care with other QT-prolonging drugs and in patients with cardiac risk factors).
