  • Route/onset: Heparin acts immediately, given parenterally (IV/SC); warfarin acts orally with a delayed onset (2–3 days, as it depends on depletion of existing clotting factors)
  • Mechanism/monitoring: Heparin acts via antithrombin III (potentiates it) and is monitored by aPTT; warfarin inhibits vitamin K epoxide reductase (reduces synthesis of factors II, VII, IX, X) and is monitored by PT/INR
