Question

Insulin analogues.

Q33 marksShort Notes

Answer

Insulin analogues are recombinant insulin preparations with modified amino acid sequences that alter their pharmacokinetic profile compared to human insulin.

Rapid-acting analogues (used with meals)

  • Insulin lispro, Insulin aspart, Insulin glulisine

Long-acting analogues (basal insulin)

  • Insulin glargine, Insulin detemir, Insulin degludec

Advantages

  • Rapid-acting: faster onset, shorter duration → better postprandial control, can be injected just before meals, less late postprandial hypoglycaemia
  • Long-acting: peakless, steady basal profile → lower risk of nocturnal hypoglycaemia, once-daily dosing
