Answer

For acute anginal attacks, sublingual Glyceryl trinitrate (GTN) is the drug of choice. Given sublingually, it bypasses first-pass hepatic metabolism, providing rapid absorption and onset of action within 1–2 minutes. It causes venodilatation (reducing preload) and coronary vasodilatation, rapidly relieving anginal pain; if pain persists after 2–3 doses at 5-minute intervals, other causes (e.g., myocardial infarction) should be considered.
