Morphine is used in acute myocardial infarction primarily for potent analgesia, which relieves severe ischemic chest pain and, importantly, reduces the pain-driven sympathetic surge (tachycardia, hypertension) that would otherwise increase myocardial oxygen demand and worsen ischemia. It also causes venodilation, reducing preload and myocardial wall tension, and relieves the anxiety associated with an acute coronary event. Used cautiously given the risk of respiratory depression and hypotension.
