Answer
Rifampicin is a rifamycin antibiotic, a first-line antitubercular drug.
Mechanism of action
- Inhibits bacterial DNA-dependent RNA polymerase, blocking RNA synthesis (bactericidal)
Uses
- Tuberculosis (first-line, part of combination therapy)
- Leprosy (multidrug therapy)
- Prophylaxis for close contacts of meningococcal meningitis / H. influenzae type B
- Carrier state elimination for meningococcus
Adverse effects
- Hepatotoxicity
- Orange-red discolouration of body secretions (harmless)
- Flu-like syndrome with intermittent dosing
- Potent hepatic microsomal enzyme induction — reduces efficacy of oral contraceptives, warfarin, corticosteroids
- Thrombocytopenia, haemolytic anaemia (rare, immunological)
- GI upset
