Rifampicin is a first-line antitubercular drug.
Mechanism of action: inhibits bacterial DNA-dependent RNA polymerase, blocking mRNA synthesis — bactericidal against both actively dividing and semi-dormant organisms, part of why it’s included in both the intensive and continuation phases of TB therapy.
Adverse effects: hepatotoxicity (additive with isoniazid’s own hepatotoxicity when combined), harmless orange-red discoloration of body fluids (urine, tears, sweat — important to pre-counsel patients about), and potent, broad-spectrum hepatic CYP450 induction — accelerating metabolism of oral contraceptives (contraceptive-failure risk), warfarin, and numerous other co-administered drugs.
Uses: tuberculosis (combination therapy, both treatment phases), leprosy (combination MDT), chemoprophylaxis for close contacts of meningococcal meningitis.
