Answer
Rifampicin is a rifamycin antibiotic, a first-line antitubercular drug.
Mechanism of action
- Inhibits bacterial DNA-dependent RNA polymerase, blocking RNA synthesis (bactericidal)
Uses
- Tuberculosis (first-line, combination therapy)
- Leprosy (multidrug therapy)
- Prophylaxis for meningococcal and H. influenzae type B contacts
- Carrier state elimination for meningococcus
Adverse effects
- Hepatotoxicity
- Orange-red discolouration of secretions
- Flu-like syndrome (intermittent dosing)
- Potent hepatic enzyme induction (reduces efficacy of OCPs, warfarin)
- Thrombocytopenia, haemolytic anaemia (rare)
- GI upset
