Answer

Rifampicin is a rifamycin antibiotic, a first-line antitubercular drug.

Mechanism of action

  • Inhibits bacterial DNA-dependent RNA polymerase, blocking RNA synthesis (bactericidal)

Uses

  • Tuberculosis (first-line, combination therapy)
  • Leprosy (multidrug therapy)
  • Prophylaxis for meningococcal and H. influenzae type B contacts
  • Carrier state elimination for meningococcus

Adverse effects

  • Hepatotoxicity
  • Orange-red discolouration of secretions
  • Flu-like syndrome (intermittent dosing)
  • Potent hepatic enzyme induction (reduces efficacy of OCPs, warfarin)
  • Thrombocytopenia, haemolytic anaemia (rare)
  • GI upset
