Answer

Sumatriptan is a selective 5-HT1B/1D receptor agonist (“triptan”), first-line for acute migraine.

MOA: sumatriptan5-HT1B/1D receptor agonism → cranial vasoconstriction and inhibition of trigeminal neuropeptide (CGRP, substance P) release → reduces the neurogenic inflammation and vasodilation underlying the migraine attack.

Uses: acute moderate-to-severe migraine attack, cluster headache.

Adverse effects: chest tightness/“triptan sensation,” flushing, paresthesia, coronary vasospasm (contraindicated in ischemic heart disease and uncontrolled hypertension), risk of serotonin syndrome with concurrent SSRIs/MAOIs.
