Question

Dopamine/Dobutamine in congestive heart failure Name the following (Two examples for each): (5x1=5)

Q150.3 marksShort Answers

Answer

Chosen drug: Dobutamine

Justification: Dobutamine is a relatively selective β1 agonist, producing a strong positive inotropic effect (increasing cardiac contractility) with comparatively less increase in heart rate and peripheral vascular resistance, making it well suited for acute decompensated heart failure/cardiogenic states where improving cardiac output without excessive afterload increase is desired. Dopamine, at the doses needed for a significant inotropic effect, also causes greater tachycardia and (at higher doses) α1-mediated vasoconstriction, which can increase afterload and myocardial oxygen demand — less favourable in heart failure management.
