Sirolimus is the appropriate choice in a patient with renal failure — unlike calcineurin inhibitors, mTOR inhibition does not share calcineurin’s renal vasoconstrictive nephrotoxic mechanism, making sirolimus a specific alternative where calcineurin-inhibitor nephrotoxicity would be a particular concern. Cyclosporine’s dose-limiting nephrotoxicity (direct renal vasoconstriction and, with chronic use, interstitial fibrosis) makes it a poor choice in a patient whose renal function is already compromised.
