Mechanism of action

  • PPIs: prodrugs that accumulate in the acidic parietal cell canaliculus, convert to their active sulfenamide form, and irreversibly bind H⁺/K⁺-ATPase, blocking the final common step of acid secretion.
  • H2 antagonists: competitively block histamine H2 receptors on parietal cells, reducing histamine-stimulated acid secretion (a less complete block than PPIs, since gastrin/acetylcholine-driven secretion is comparatively spared).

One H. pylori eradication regimen: PPI (Omeprazole) + Clarithromycin + Amoxicillin, for 10-14 days.
