Route/spectrum: Ketoconazole is used mainly topically/for superficial infections and has significant hepatotoxicity limiting systemic use; fluconazole has good oral bioavailability, excellent CSF penetration, and is preferred for systemic infections including cryptococcal meningitis
Drug interactions/toxicity: Ketoconazole is a stronger CYP450 inhibitor with more drug interactions and greater hepatotoxicity risk; fluconazole has fewer interactions and a better safety profile
