The germ tube test is a rapid, simple presumptive identification test for Candida albicans (and the closely related C. dubliniensis), distinguishing it from most other clinically significant Candida species.
Principle: when yeast cells of C. albicans are suspended in a protein-rich medium (classically human or animal serum) and incubated at 37°C for 2–3 hours, they produce a characteristic germ tube — a thin, filamentous outgrowth from the yeast cell that lacks a constriction at its point of origin (unlike a true pseudohypha, which shows a distinct constriction/septum where it joins the parent cell) — this specific morphological detail (absence of constriction) is the defining diagnostic feature distinguishing a true germ tube from early pseudohyphal formation seen in some other Candida species.
Procedure: a small inoculum of the test yeast colony is suspended in 0.5 mL of serum, incubated at 37°C, and examined microscopically at 2 and 3 hours for the presence of germ tubes.
Interpretation: presence of germ tubes = positive, presumptively identifying C. albicans (or C. dubliniensis, which cannot be distinguished from C. albicans by this test alone and requires further biochemical/molecular differentiation if needed); absence of germ tubes = negative, suggesting a non-albicans Candida species (e.g., C. glabrata, C. krusei, C. tropicalis), which require further testing (culture morphology, biochemical panels, chromogenic media, or molecular methods) for definitive species identification.
Clinical significance: rapid, low-cost, and widely used as a first-line presumptive test in clinical microbiology laboratories, given the clinical importance of promptly distinguishing C. albicans (generally more predictably antifungal-susceptible) from non-albicans species (which show more variable and sometimes intrinsic antifungal resistance, e.g., C. krusei’s intrinsic fluconazole resistance), directly influencing empirical antifungal therapy decisions while definitive identification/susceptibility testing is pending.
