Answer
Propranolol is a non-selective beta-adrenergic antagonist (blocks both β1 and β2 receptors), historically the prototype beta-blocker.
Uses
- Hypertension (reduces cardiac output and renin release)
- Angina pectoris (↓heart rate and contractility, ↓myocardial O₂ demand)
- Cardiac arrhythmias (supraventricular tachycardia, control of ventricular rate in atrial fibrillation)
- Post-myocardial infarction (reduces reinfarction and mortality)
- Migraine prophylaxis
- Essential tremor
- Anxiety (performance/situational anxiety — controls somatic symptoms)
- Hyperthyroidism (symptomatic control, and inhibits peripheral T4→T3 conversion at high doses)
Adverse effects
- Bradycardia, heart block
- Bronchospasm (β2 blockade — contraindicated in bronchial asthma)
- Fatigue, cold extremities (reduced peripheral blood flow)
- Masking of hypoglycemia symptoms in diabetics
- Rebound hypertension/angina on abrupt withdrawal
- Erectile dysfunction, vivid dreams/depression
