Question
Sublingual administration of drugs
Q91 marksShort Answers
Answer
The sublingual mucosa is thin and richly vascularized, allowing rapid drug absorption directly into the systemic (superior vena cava) circulation, bypassing hepatic first-pass metabolism entirely. This makes the route ideal for drugs with high first-pass metabolism or those needing a rapid onset of action — e.g. glyceryl trinitrate for acute anginal attacks, where onset within 1–2 minutes is clinically essential and oral bioavailability would otherwise be destroyed by first-pass hepatic metabolism.
