Clarithromycin, an azalide/macrolide antibiotic.
Mechanism of action
- Binds reversibly to the 50S ribosomal subunit, inhibiting bacterial protein synthesis by blocking translocation
Uses
- Respiratory tract infections
- H. pylori eradication (part of triple therapy)
- Mycobacterium avium complex prophylaxis/treatment in HIV
- Alternative to penicillin in penicillin-allergic patients
Adverse effects
- GI upset (less than erythromycin)
- QT prolongation
- Hepatotoxicity (rare)
- CYP450 inhibition — multiple drug interactions
