Ethylene oxide (ETO) sterilization A gaseous chemical sterilization method used for heat-sensitive medical equipment (plastics, electronic devices, delicate instruments) that cannot withstand autoclaving. ETO is a highly penetrating, alkylating agent that kills microorganisms (including bacterial spores) by alkylating nucleic acids and proteins, disrupting normal cellular metabolism and replication. The process requires controlled temperature (typically 37–63°C), humidity, gas concentration, and exposure time, followed by a mandatory prolonged aeration period to remove toxic residual gas before the item can be safely used, since ETO is flammable, explosive, carcinogenic, and irritant to tissues — an important occupational safety/biosafety consideration. Used for catheters, endoscopes, sutures, and other single-use/heat-sensitive disposables. Sterility monitoring uses chemical indicators and biological indicators (Bacillus atrophaeus spores).

Plasma sterilization (hydrogen peroxide gas plasma) A modern, low-temperature sterilization method in which vaporized hydrogen peroxide is subjected to a strong electromagnetic field (radiofrequency or microwave energy) within a vacuum chamber, generating a low-temperature gas plasma containing highly reactive free radicals (hydroxyl and hydroperoxyl radicals) that rapidly kill microorganisms by damaging their cell membranes, enzymes, and nucleic acids. The process operates at low temperature (45–55°C) and produces only water vapour and oxygen as by-products, leaving no toxic residue and requiring no aeration period — a major advantage over ETO. Used for sterilizing heat- and moisture-sensitive instruments (e.g., endoscopes, some electronic surgical devices) with a much shorter overall cycle time (~45–75 minutes) than ETO (which can take many hours including aeration).

Comparison: both are low-temperature sterilization methods suited to heat-sensitive items, but plasma sterilization is faster, leaves no toxic residue, and avoids ETO’s flammability/carcinogenicity concerns, making it an increasingly preferred modern alternative where available, though ETO remains widely used, particularly for items with complex lumens that plasma may not adequately penetrate.
