Answer

Colchicine is the appropriate choice for an acute gout attack — it inhibits neutrophil microtubule polymerization and migration into the joint, rapidly reducing the inflammatory response to deposited urate crystals. Allopurinol, a xanthine oxidase inhibitor that lowers serum urate, is unsuitable for an acute attack: starting it during a flare can mobilize urate crystals and worsen or prolong the attack. Allopurinol is reserved for interval (prophylactic) therapy, started only after the acute attack has resolved.
