Major types of Hospital-Acquired Infections (HAI):
- Catheter-Associated Urinary Tract Infection (CAUTI) — the most common HAI in many settings, associated with indwelling urinary catheters; commonly E. coli, Klebsiella, Pseudomonas.
- Surgical Site Infection (SSI) — infection at or near a surgical incision within a defined post-operative period; commonly S. aureus (including MRSA), coagulase-negative staphylococci.
- Central Line-Associated Bloodstream Infection (CLABSI) — associated with intravascular catheters; commonly coagulase-negative staphylococci, S. aureus, Candida.
- Ventilator-Associated Pneumonia (VAP) — pneumonia developing in mechanically ventilated patients; commonly Pseudomonas aeruginosa, Acinetobacter, Klebsiella.
- Clostridium difficile-associated diarrhoea — following antibiotic-disrupted normal gut flora.
Risk factors: invasive devices, prolonged hospital stay, immunosuppression, extremes of age, broad-spectrum antibiotic use, surgical procedures.
Note on standard precautions Standard precautions are a set of infection-control practices applied to all patients regardless of known/suspected infection status, since any patient may harbour a transmissible pathogen:
- Hand hygiene — the single most important and cost-effective measure, before/after every patient contact.
- Personal Protective Equipment (PPE) — gloves, gowns, masks, eye protection, used based on anticipated exposure to blood/body fluids.
- Safe injection practices — single-use needles/syringes, safe sharps disposal.
- Respiratory hygiene/cough etiquette.
- Environmental cleaning and disinfection of surfaces/equipment.
- Safe handling and disposal of biomedical waste, including sharps.
- Appropriate patient placement/isolation when transmission-based precautions (contact, droplet, airborne) are additionally required for specific known/suspected infections, building on the foundation of standard precautions applied universally.
Significance: consistent application of standard precautions, combined with antibiotic stewardship and device-minimization strategies, forms the core of hospital infection-prevention programmes, given the significant morbidity, mortality, and cost associated with HAIs.
