Non-neural vaccines for rabies

Non-neural (modern cell-culture-derived) rabies vaccines are the current standard for rabies prophylaxis, having replaced the older, neural-tissue-derived vaccines (e.g., Semple vaccine, prepared from infected sheep/goat brain tissue) that carried a significant risk of severe neuroparalytic adverse reactions due to contaminating host myelin/neural antigens (causing an autoimmune-type post-vaccinal encephalomyelitis).
Types:
- Human Diploid Cell Vaccine (HDCV) — rabies virus grown in human diploid cell culture (e.g., WI-38 or MRC-5 cell lines), then inactivated; highly immunogenic and safe, but relatively expensive to produce.
- Purified Chick Embryo Cell vaccine (PCECV) — rabies virus grown in primary chick embryo fibroblast cell culture, then purified and inactivated; widely used, cost-effective alternative to HDCV.
- Purified Vero Cell Rabies Vaccine (PVRV) — grown in Vero cell line (a continuous monkey kidney cell line); widely used, particularly in many national immunization programmes given good safety/efficacy profile and relatively lower cost.
- Purified Duck Embryo Vaccine (PDEV) — grown in duck embryo, an older cell-culture-based option, less commonly used now.
Advantages over neural-tissue vaccines: markedly improved safety profile (no risk of neuroparalytic reaction, since no host neural tissue/myelin is present), higher immunogenicity/potency, and shorter, more convenient dosing schedules — for these reasons, WHO recommends cell-culture-derived vaccines as the standard of care, and neural-tissue vaccines (Semple vaccine) have been phased out in most countries, including India.
Administration: given intramuscularly (deltoid, or anterolateral thigh in infants) or, increasingly, by the more cost-effective intradermal route (using a fraction of the intramuscular dose at multiple sites), following a defined post-exposure prophylaxis schedule (e.g., days 0, 3, 7, 14, ± 28), alongside wound care and Rabies Immunoglobulin for Category III exposures.
