Answer

Furosemide is a loop diuretic.

Mechanism of action

  • Inhibits the Na⁺-K⁺-2Cl⁻ (NKCC2) cotransporter in the thick ascending limb of the loop of Henle
  • Abolishes the medullary concentration gradient, causing marked natriuresis, kaliuresis, and loss of urinary concentrating ability

Uses

  • Acute pulmonary oedema/left ventricular failure
  • Chronic congestive cardiac failure
  • Oedema (hepatic, renal, nephrotic syndrome)
  • Acute renal failure
  • Hypertension (with renal impairment)
  • Hypercalcaemia

Adverse effects

  • Hypokalaemia, hyponatraemia, hypomagnesaemia
  • Hypotension, dehydration
  • Ototoxicity
  • Hyperuricaemia
  • Hyperglycaemia
  • Metabolic alkalosis
