Carbamazepine is the appropriate choice — the drug of choice for trigeminal neuralgia, its Na⁺ channel-blocking action suppressing the paroxysmal, high-frequency neuronal firing that underlies the characteristic lancinating pain. Phenobarbitone, while an antiepileptic, has no established specific efficacy in trigeminal neuralgia and is not used for this indication.
