Answer

Thalidomide has immunomodulatory, anti-inflammatory, and anti-angiogenic properties.

Mechanism of action

• Inhibits TNF-alpha production and has anti-angiogenic effects
• Exact mechanism of teratogenicity not fully elucidated, but thought to involve inhibition of angiogenesis in the developing limb bud

Uses

• Erythema Nodosum Leprosum (Type 2 lepra reaction) — drug of choice
• Multiple myeloma
• Refractory graft-versus-host disease

Adverse effects

• Severe teratogenicity (phocomelia) — absolutely contraindicated in pregnancy/women of childbearing potential without strict contraception
• Peripheral neuropathy
• Sedation
• Constipation
• Increased risk of venous thromboembolism
