Answer
Thalidomide has immunomodulatory, anti-inflammatory, and anti-angiogenic properties.
Mechanism of action
- Inhibits TNF-alpha production and has anti-angiogenic effects
- Exact mechanism of teratogenicity not fully elucidated, but thought to involve inhibition of angiogenesis in the developing limb bud
Uses
- Erythema Nodosum Leprosum (Type 2 lepra reaction) — drug of choice
- Multiple myeloma
- Refractory graft-versus-host disease
Adverse effects
- Severe teratogenicity (phocomelia) — absolutely contraindicated in pregnancy/women of childbearing potential without strict contraception
- Peripheral neuropathy
- Sedation
- Constipation
- Increased risk of venous thromboembolism
