This is the classic Beck’s triad of cardiac tamponade: hypotension (reduced cardiac output from impaired ventricular filling), muffled/distant heart sounds (fluid in the pericardial space), and elevated jugular venous distension (impaired venous return to the heart). Cardiac tamponade is a feared complication of MI due to myocardial rupture with haemopericardium.
