Alcoholic cirrhosis is the end-stage of chronic alcohol-related liver disease, representing irreversible fibrosis and nodular regeneration.
Macroscopic (gross) changes
- Initially, the liver may be enlarged (fatty change), but with progression to cirrhosis it becomes shrunken and firm
- Diffusely nodular surface, with small, fairly uniform-sized regenerative nodules (micronodular cirrhosis, classically <3 mm), giving a “hobnail” appearance
- Yellow-tan colour (if fatty change persists) or brown (with progressive fibrosis)

Microscopic changes
- Bands of fibrous septa (collagen) surrounding regenerative nodules of hepatocytes, disrupting the normal lobular architecture
- Micronodular pattern — relatively uniform, small regenerative nodules (a hallmark of alcoholic cirrhosis, as opposed to the larger, more variable nodules of macronodular/post-hepatitic cirrhosis)
- Residual features of alcoholic liver disease may coexist: hepatocyte steatosis (fatty change), Mallory-Denk bodies (eosinophilic cytoplasmic inclusions of damaged intermediate filaments), ballooning degeneration, and neutrophilic infiltration (features of alcoholic hepatitis, if superimposed)
- Progressive loss of normal architecture with nodule-septa pattern throughout the liver
