Sporicidal disinfectants

Sporicidal disinfectants are chemical agents capable of killing bacterial spores, not merely vegetative organisms — a much more demanding standard than ordinary disinfection, required for “high-level disinfection” of heat-sensitive medical equipment that cannot be autoclaved.
Examples:
- Glutaraldehyde (2% activated alkaline solution) — a widely used high-level/sporicidal disinfectant for heat-sensitive medical/surgical equipment (e.g., endoscopes, bronchoscopes); requires prolonged contact time (several hours) for full sporicidal activity, versus a shorter exposure for high-level disinfection against vegetative organisms.
- Formaldehyde — used as an aqueous solution or as a gas for room/equipment fumigation; sporicidal with adequate concentration/exposure, but pungent, irritant, and a suspected carcinogen.
- Hydrogen peroxide (high concentration, e.g., 6–7.5%) and peracetic acid — modern alternatives with sporicidal activity and less toxic residue than glutaraldehyde/formaldehyde.
- Chlorine-releasing compounds (sodium hypochlorite at high concentration) — sporicidal at higher concentrations/longer exposure than needed for ordinary bactericidal disinfection.
- Ethylene oxide gas — a gaseous sterilant, sporicidal, used for heat-sensitive medical devices.
- Iodine/iodophors at high concentration can show some sporicidal activity, though generally less reliable than the agents above.
Clinical relevance: sporicidal disinfectants are essential wherever true sterilization (autoclaving) is not possible for a given item, but where the item may be contaminated with spore-forming organisms (e.g., endoscopic equipment used on patients with Clostridium difficile or suspected prion disease) — ordinary “low-level” or “intermediate-level” disinfectants (e.g., alcohols, many quaternary ammonium compounds) are NOT sporicidal and are inadequate in these situations.
